Mountain Vista Senior Living, an American Baptist Homes of the Midwest community, is looking for a knowledgeable and compassionate Social Services Director (LSW or LCSW) to join our team. - Pay from $64,500 based on experience.
- This is a full-time day shift position working 8am-430pm.
As the Social Services Director you will provide a high level of customer service to residents and family members; conduct SS assessments, and social histories, work with community agencies and update care plans and MDS. Part of your role will be to address resident and family concerns, plan discharges, handle abuse investigations and finalize State reports. Lastly, you will manage a team of social workers.
What you will need:
- Must have knowledge of Social Security, Medicare and Medicaid.
- Bachelor's Degree in Human Services or Bachelor's degree in Social Work from an accredited school of social work required. Master's preferred.
- Licensed Social Worker (LSW) required. LCSW preferred.
- Social work experience required; long-term care experience required.
- Supervisory experience required.
