FTE: 1.0; Shift: Day; Schedule: N40 Position SummaryThe Home Care Coordinator Case Manager (HCC) assesses the in-home care needs for PACE participants and coordinates with internal and external resources to ensure appropriate care is delivered. The HCC provides in-home case management to PACE participants and provides oversight to home care aides to ensure that the participants in-home plan of care is effectuated. Responsibilities
Assesses in-home care needs by doing an in-person assessment in the participant’s home on admission and routinely thereafter
Coordinates appropriate care with internal and external care providers (home care aides)
Provides oversight to the plan of care by observing care and reviewing records
Coordinates with compliance to ensure that caregivers are following the plan of care
Assess for change of condition and completes new assessments as needed
Provides case management for in-home care needs for supplies/equipment and supports changes to plan of care when there is a change in participant status, i.e. acute illness or injury
Coordinates with the Inter-Disciplinary Team to ensure that the established plan of care is effectuated
Requirements
Two years of experience in case management or care coordination in healthcare setting or community agency
Advanced degree in health field preferred i.e. MSW
License in health field preferred i.e. PTA, COTA, OT, PT, LCSW, LPN, RN

Pay and Benefit ExpectationsWe provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $26.04 - $37.47 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education.
