We're seeking a visionary and dynamic leaders who are creative in problem solving and contribute new ideas and solutions to the organization to join our team in Integrated Clinical Services. The Behavioral Health Provider (BHP) position provides direct clinical services to Primary Care patients with a focus on providing transitions of care support for patients who have been discharged from a higher level of care. This includes complex care coordination with the primary care team, hospitals, and community providers to assist patients with connecting to care. The BHP will develop a detailed post discharge support plan with the patient, in order to meet their needs, consider all care options and settings, appropriate level of service, and risk mitigation plan. BHP's will also assist with care coordination for patients on the suicide high risk list and will partner with other BHP's to support patients with complex mental health issues. BHP's will use process-oriented sessions to support a patient around learning new coping skills. These sessions may extend beyond the traditional Integrated Behavioral sessions of 4-6 sessions due to the patient's complex needs. This position will require employees to provide patient support throughout several clinics within our Community Health Clinics. The position also requires Qualified Mental Health Associate (QMHA), as the position is located in the Multnomah County Community Health Center, which is a covered Federally Qualified Health Center (FQHC) under the Bureau of Primary Health Care and the Centers for Medicare and Medicaid Services of the United States Department of Health and Human Services. The position creates a work culture of respect, trust and understanding for the highly diverse populations the Community Health Department serves in the office and in the community. This position will be based at the Mid County Health Center and will also provides services to the East County, Rockwood and possibly all Multco Clinics depending on need.
As The Behavioral Health Provider, Other Duties IncludeTransitions of Care
  • Provide complex support for patients transitioning from acute care settings.
  • Assist with discharge planning, identifying behavioral health supports, and assist in connection to prescriber who can determine medication need and effective management.
  • Continue to track and coordinate this work until the patient has been stabilized and connected to the appropriate level of care.
  • Complete assessment and evidenced-based psychotherapy interventions to Primary Care patients across the lifespan with medical conditions to support their management of recovery from these conditions. Inquire of current and past events, preference of care plan delivery, partnering with staff at the client level for a team based approach, document decisions and next steps in the care process.
  • Applies knowledge and experience in the relationship between medical and behavioral health aspects of health and disease while engaging in consultation with patients and primary care team members.
  • This position may utilize a hybrid of in person visits and/or telehealth/telemedicine model of care.
Mental Health and Health Compromising Behavior Care
  • Provide mental health assessment, diagnosis, and evidenced-based process oriented psychotherapy to Primary Care patients with co-occurring disorders across the lifespan.
  • Utilizing a trauma informed approach, work with clients to improve mental health through processing past events, learning new coping skills, and managing their complex mental health and co-occurring disorder needs.
  • Continuity care is more frequent and continuous to a small number of high needs patients in the population.
  • Provide direct assessment and intervention for threatening and/or suicidal clients; assess client's level of suicide risk, cognitive functioning, and other safety and risk factors.
  • Evaluation of specific case plans to meet medical case management and psycho-social needs of patients, including those receiving medication supported recovery (MSR).
  • Perform advanced, comprehensive behavioral health services involving assessment and analysis of complex psycho-social factors.
Consultation
  • Provide mental health consultation and education to the Primary Care Provider teams regarding psychosocial treatments and specific behavioral issues or barriers that arise related to a patient's health issues.
  • Partner with other providers and staff for each client and his/her/their needs to include family members and any other support systems for the client.
  • Provides timely and succinct feedback to other Primary Care Providers regarding consultation findings and recommendations.
  • Formulates diagnostic and treatment recommendations and presents findings to treatment teams. Teaches Primary Care team members about behavioral health care topics and clinical issues.
  • Read current journals and publications to keep current on consultation and care trends to include culturally based care, community interactions and connections, etc.
Case Management
  • Provide complex care coordination and case management services to patients with complex mental health and co-occurring disorder needs with the goal of improving the patient's overall stability.
  • Maintain relations and general knowledge of psychiatric hospitals, available community mental health and behavioral health services, and substance use treatment services.
  • Maintain up to date notes and files per client event and interaction.
  • Work closely with the Primary Care team's community health worker to address social determinants of health.
Documentation and Quality Improvement
  • Documents all work with and on behalf of patients in the electronic health record, including behavioral health visits, telephone encounters, and other work.
  • Completes documentation within stated time lines.
  • Participates in quality improvement activities that support the Integrated Behavioral Health team, utilizes relevant MH data in decision making and planning.

  • Training and experience for this position is Board Certification and State licensure as a Nurse Practitioner or Physician Assistant and one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position.
