This position is responsible for assessing, coordinating and teaching the individual with diabetes and their support person to manage their disease. The dietitian works closely with physicians, nursing staff and Diabetes Center (DC) team to plan and provide quality care to patients with diabetes. Manages all the nutrition aspects of diabetes patient care and works with DC team designing curriculum and classes relating to Diabetes Mellitus. The Senior Certified Diabetes Dietitian has developed a deeper understanding of diabetes care and has the ability to teach patients and modify practice to various situations, including insulin pump therapy, insulin titration, continuous glucose sensing and diabetes injectables.
Responsibilities:(45%)*Assesses inpatient and outpatients educational needs, develops training/education plans, and instructs patients ranging in age from infant to 100 years. Creates an individualized diabetic meal plan with family/support system incorporating principles of diabetes control. Documents appropriately in the patient chart.
(20%)*Participates in the education and training of patients on Continuous Glucose Monitoring Systems, Insulin Pumps and their software.
(20%)*Develops and implements comprehensive diabetes self-management programs, classes and clinics that are of interdisciplinary nature, including insulin pump clinic.
(15%)*Provides education on the use of diabetes devices and medications, including insulin and other injectable medications. Uses Staged Diabetes Management as Diabetes Center protocol for insulin adjustment.
OTHER DUTIES AND RESPONSIBILITIES:
Participates in special projects such as planning and implementation of professional educational conferences, community education, support groups, detection programs and marketing endeavors. Supervises dietetic interns. Provides diabetes education experience. Participates in Diabetes Center Meetings, including staff meeting, interdisciplinary patient care conference (PCMH) and meetings with providers. Supervises screening of inpatients with diabetes or hyperglycemia performed by the dietetic technician for nutritional status and educational needs. Stays current with research and trends and serves as a resource for inquiries related to the specialty area. Other duties as assigned.
Certifications and Licensures:CDR - Commission on Dietetic Registr Required
Equivalent Experience - Registered Dietitian status with the CDR (Commission on Dietetic Registration) for a minimum of three years required.
Required Education:Bachelor's Degree
Comments: Bachelor's Degree in Science or Art with knowledge in nutrition required. Three years clinical or community dietetic experience required
Other Information:EXPERIENCE, TRAINING AND SKILLS:
Certification as Diabetes Educator and maintenance of certification with at least 15 hours of continuing education per year preferred. Certification on at least one insulin pump, with maintenance of certification with changing of technology required. Certification in at least one continuous glucose monitoring system and maintenance of certification with upgrades required. Interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understandable manner.
