This position is responsible for assessing, coordinating and teaching the individual with diabetes and/ortheir support person to manage their disease. The nurse works closely with physicians, nursing staff andDiabetes Center (DC) team to plan and provide quality care to diabetic patients. Manages all the nursingaspects of diabetic patient care and works with DC team designing classes and clinics relating to DiabetesMellitus.
Responsibilities:(35%)*Assesses inpatient and outpatients educational needs, develops and instructs patients ranging inage from toddler to 100 years and implements a plan of care for diabetes education. Documentsappropriately in the patient chart.(35%)*Engages in inpatient and outpatient/family teaching (group & individual) as appropriate. Evaluatesthe patient's responses to the teaching and adjusts the education style and plan to meet the needs of thepatient.(15%)*Uses Staged Diabetes Management Decision Paths as Diabetes Center protocol for insulinadjustment.(15%)*Teaches new technology to patients, i.e. Continuous Glucose Monitoring, Insulin Pumps, Inject ablemedications, Insulin delivery devices and blood glucose meters
OTHER DUTIES AND RESPONSIBILITIES:Develops and implements comprehensive diabetes selfmanagement programs and classes that are of interdisciplinary nature.Participates in special projects such as set-up of professional educational conferences, communityeducation, support groups, detection programs and marketing endeavors.Plans and implements continuing education and/or reviews education content as needed for MyMichiganHealth staff including inpatient nursing staff, providers and outpatient medical office staff.Participates in Diabetes Center Staff, Program and Advisory meetings. Actively participates with physicianand health professionals in conducting patient group classes and clinics, including insulin pump clinic.All other duties as assigned.
Certifications and Licensures:CDE - Cert Diabetes EducatorPreferred Equivalent Experience - Certified Diabetes Educator preferred, withmaintenance of certification with at least 15 hours of continuing education peryear. If no certification, passing of test within three years of hire is expected.
