Description

Job Title: Case Manager 

Department: Adult Services 

Reports To: Director of Person-Centered Supports 

FLSA STATUS: Non-Exempt 

General Purpose of the Job: The Case Manager (CM) oversees the person-centered planning process (PCP) and advocates for individuals served in a holistic approach. The CM acts as an agency representative and interfaces with LTSS and the state to ensure that gaps or errors in services/funding are addressed and adjusted appropriately while advocating for person-centered supports for each person served. 

Additionally, the CM advises the team on funded staffing, approved through agreement in the team meeting. This position is responsible for monitoring and accepting funding as approved through the legacy PCIS2 billing and through LTSS. Additionally, the CM maintains a template of hours and/or units approved by the state for each person served on their caseload. The CM accesses the state and federal database system daily and makes updates to reflect discharges, intakes, and additions of staffing support units and/or hours. Daily monitoring and administration in PCIS2 (until defunct) and LTSS are required. 

Essential Duties and Responsibilities: 

  1. Leads and supports the person-centered planning process by ensuring facilitation, implementation, training, compliance, and monitoring of the PCP process. 

  2. Advocates for and ensures all services are person-centered and reflect the desires of persons served. 

  3. Coordinates with the CCS to schedule PCP meetings 60-90 days before the annual PCP meeting. 

  4. Participates in ongoing development of a person-centered process to meet desired outcomes and goals while adhering to service and funding requirements. 

  5. Ensures annual PCP goals are relevant and measurable and that supplemental assessments are complete (including but not limited to the following assessments: Unsupervised time, self- medicating, personal spending). 

  6. Informs the delegating nurse of the upcoming PCP so they can prepare to review the HRST. 

  7. Supports other team members through review of the PCP packet, reviewing PCP goal and programmatic content. 

  8. Reviews details of service plans and ensure that all information is current and updated daily in PCIS2/LTSS/ICM software. 

  9. Daily monitors LTSS and PCIS2 to verify that funding is in place to support service recipients. 

  10. Authorizes services and changes in services in PCIS2/LTSS software according to the approved PCP. 

  11. Saves approved PCP’s into ICM database and assists with training on plans, as appropriate. 

  12. Notifies NCIA’s Fiscal Department in changes to services. 

  13. Reviews referrals in LTSS software and makes decisions on admission within 5 business days or as deadline requires (3 days, 1 day). 

  14. Advises program director of funding changes that affect staffing (reduction or addition of dedicated hours, addition of 1:1 staffing, shared hours, etc.). 

  15. Informs teams of start and end dates, once established by the state. 

  16. Attends work regularly according to assigned work schedule and in accordance with Agency policy. 

  17. Works collaboratively with Admissions, Referrals, and Discharge Manager and provides follow-up for referrals. 

  18. Works collaboratively with other team members to develop and implement plans of correction for the Office of Health Care Quality (OHCQ). 

  19. Works and communicates cooperatively with all staff, supervisors, administrators, co-workers, residents served, community professionals, customers, vendors, and the public. 

  20. Ensures Community Setting Rule requirements related to the PCP are met. 

  21. Assumes all other duties and responsibilities as required.

Qualifications


Education: 

  • Bachelor of Arts or Science degree in the areas of social work, counseling, psychology, special education or related field. 

  • Must have reliable transportation and an acceptable driving record as determined by the criteria established by the Agency's insurance carrier and Agency policy. 

Experience: 

  • Two years’ experience working with residents with intellectual disabilities, plus two years’ experience in case management and developing resident plans of service is required. 

Language Skills: 

  • Ability to read and comprehend simple instructions, short correspondence, and memos. 

  • Must have excellent writing skills. 

  • Ability to facilitate meetings and communicate effectively with a diverse group. 

Mathematical Skills: 

  • Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. 

  • Ability to apply concepts of basic algebra and geometry. 

Reasoning Ability: 

  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. 

  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. 

Certificates, Licenses, Registrations: 

  • Obtain and remain current in MANDT, and CPR/FA certifications. 

Other Skills and Abilities: 

  • Ability to interact positively with others to achieve goals for program and clients. 

  • Proficient in Microsoft Office Suite. 

  • Ability to facilitate small meetings. 

  • Solid organizational skills. 

  • Time management skills. 

  • Multi-tasking ability. 

  • Ability to meet deadlines. 

  • Uses Technology for completion of specified job duties, including agency, state and federal database systems. 

Physical Demands: 

  • This position is primarily a sedentary one with frequent use of arms and hands to reach and write. 

  • Some standing and walking or ambulatory ability is required to be able to perform in this job. 

  • Ability to communicate and hear clearly as well as close vision is essential. Ability to lift 10 pounds unassisted. 

Work Environment: 

  • Primarily inside office environment and in team meetings. 

  • Work environment may include video and/or audio monitoring.
