Nationwide Children's Hospital is Hiring a Hep C Coordinator- HIV 340B Program Near Columbus, OH

Overview

This is a full-time, working 40 hours per week, exempt role.

Minimum pay rate: $34.00

Clinic hours: 8a-4:30p

The Hep C and PEP Care Coordinator provides nursing care and care coordination services for pediatric, adolescent, and adult patients who may have or have been exposed to Hepatitis C or HIV. Ensures the provision of quality patient care in the Immunodeficiency Clinic through care coordination, case management, utilization management, and education.

Responsibilities

Provides nursing care using a Family Centered approach (Care coordination for this population includes screening, assessment, counseling and testing, care plan development, communication, self-management skill introduction, collaboration between caregivers, diagnostic testing, insurance, and follow up). 

Communicates and collaborates with primary care physicians, specialty physicians, and other service providers to identify patients in need of care coordination and transitions to other services as needed.

Works closely to guide families in optimizing the care and services they receive for improved health, decreased costs, and improved satisfaction with care.

Administers treatments in a manner that improves clinical outcomes and reduces risk or cost.

Ensures delivery of preventive services, health promotion activities, and other interventions that proactively address potential risks.

Aware of evidence-based standards and applicable national standards.

Participates in policy and procedure development, continuing education, continuous quality improvement, and research and/or evidenced based practice.

Develops and implements educational programs for patients, parents and families, hospital staff, and community health personnel.

Engages in the “workplace community” by staying aware of changes, participating in data collection or auditing, understanding Quality Improvement, and representing staff at relevant meetings and providing resources.

Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses.

Qualifications

Education: BSN.Licensure Requirement:• Licensed as a registered nurse in the State of Ohio.• Valid driver’s license, good driving record and proof of insurance.Experience: Clinical skill and knowledge in the nursing care of children including normal growth and development and/or clinical expertise in specialized area required.The above list of duties is intended to describe the general nature and level of work performed by individuals assigned to this classification.
