Native American Connections is Hiring a Housing Case Manager Near Surprise, AZ

Surprise Bridge Housing Location 
Schedule: Monday - Friday 8 AM to 5 PM
POSITION SUMMARY:
The primary responsibility of a housing case manager is to provide case management and navigation services to program participants. The case manager is responsible for ensuring a welcoming, safe, and professional environment. They receive and review homeless housing referrals from the Director Homeless Programs. The case manager will be responsible for explaining and accompanying the program participant through the initial move-in, orientation of program guidelines, and community rules. Case management services are to work alongside the participant to identify and secure permanent housing. The ideal candidate will have current knowledge of the Maricopa Regional Continuum of Care (CoC), Coordinated Entry System (CES), experience with housing services in Maricopa County, case management practices, and experience providing case management services to high acuity
participants. The housing program case manager is required to complete the responsibilities of the position in the field and on-site.
RESPONSIBILITIES:
  • Conduct outreach and identify individuals who meet the requirements for the housing program by using the approved assessment tool.
  • Ensures participants understand the move-in process, the program guidelines, community rules, case management services, and property rules by the state and federal laws.
  • Completes initial Individualized Service Plan (ISP) within 5 days of move-in with SMART goals to ensure housing stability.
  • Completes intake forms with participants and evaluates the level of need for workforce and case management services within 5 days of move-in.
  • Completes and updates ISP and in HMIS weekly; completes at least one in-person case management/ home visit per month.
  • Delivers and/or facilitates life-skills development curriculum for individuals and groups. Connects clients to community resources that offer life skills and employment assistance.
  • Conducts monthly housing program meetings with all program participants to address health/safety issues and concerns.
  • Addresses participants conduct or concerns with participants as needed.Provides referrals for emergency medical, behavioral health and housing stabilization as needed; provide referrals for and/or deliver financial education; takes requests and picks up food boxes; manage bus pass program; refer (and follow-up) to DES, applications for food stamps, and AHCCCS as needed; Work individually with clients to remove barriers to employability and permanent housing.
  • Enrolls and terminates participants per agency program requirements.
  • Maintains timely, accurate written file documentation on participant attendance to case management services and progress, rental history, medical and BH instances, all referrals, and recertification of income.
  • Maintains a shift report in Microsoft Excel that captures interactions with guests, vendors, and outside service providers.
  • Schedules and distributes food from the food pantry.
  • Issue Guideline notices as needed when Program Guidelines are not followed.
  • Participate in weekly staffing by reporting status updates on assigned participants.
  • Maintains an Outlook calendar to reflect meetings and tasks.
  • Participates in housing-based community meetings approved by the Director of Housing Programs.
  • Creates and conducts street outreach strategies to target specific areas. Maintains outreach reporting of contacts made and outcomes in real-time.
  • Other duties as assigned.
Educational Requirements
  • Bachelor’s degree preferred
Work Experience / Skills Requirement
  • Two years of experience in service delivery or any combination of related education, professional training or work experience which demonstrates the ability to successfully perform duties.
  • Must be comfortable working independently.
  • Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors.
  • Must demonstrate critical thinking, problem-solving, organizational and time management skills.
  • Excellent mediation and conflict resolution skills.
  • Excellent customer service skills.
  • Experience in working with the Native American population preferred. Knowledge of the economic, educational, and social problems of Native Americans and referral services
  • Excellent communication skills – written and oral
  • MS Office skills
  • Possess and maintain a valid Arizona driver’s license, reliable transportation and a clean driving record.
  • Able to perform general janitorial duties to include bending, lifting, heavy and light cleaning,
  • negotiate stairs and carry up to 50 lbs.
