Shift: Days; Monday-Friday 40 hours guaranteed, 8:00 AM - 5:00 PM
Start Date: July 29, 2024Job Summary

As a Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) in the Family Medicine unit, you will provide high-quality patient care in a clinical setting. You will work closely with physicians and other healthcare professionals to support patients of all ages with a variety of health conditions. Your role includes direct patient care, administrative tasks, and collaboration with the healthcare team to ensure the best outcomes for patients.

Essential Job Functions
  1. Patient Care:

    • Provide direct patient care including assessments, medication administration, and wound care.
    • Assist patients with daily living activities, including bathing, dressing, and mobility.
    • Perform routine medical procedures such as blood draws, injections, and vital signs monitoring.
    • Document patient care and progress in the Electronic Medical Record (EMR) system (EPIC).
  2. Clinical Support:

    • Prepare patients for examinations, procedures, and treatments.
    • Assist physicians and other healthcare professionals with patient care tasks.
    • Educate patients and families on health conditions, treatment plans, and self-care techniques.
  3. Administrative Duties:

    • Schedule patient appointments and manage patient flow.
    • Maintain patient records, including medical histories and treatment plans.
    • Coordinate with other healthcare providers and support staff to ensure comprehensive patient care.
  4. Communication and Coordination:

    • Communicate effectively with patients, families, and healthcare team members.
    • Participate in team meetings and contribute to care planning discussions.
    • Address patient concerns and provide compassionate, responsive care.
  5. Compliance and Quality Assurance:

    • Adhere to facility policies, procedures, and regulatory requirements.
    • Maintain patient confidentiality and follow HIPAA regulations.
    • Ensure quality of care and perform tasks in a manner that supports patient safety and comfort.
Skills and Abilities
  • Ability to read, write legibly, and communicate effectively in English.
  • Proficient in using computer systems and EMR (EPIC).
  • Strong clinical skills and the ability to perform medical procedures with precision.
  • Demonstrated ability to handle emergency situations calmly and efficiently.
  • Good organizational skills and attention to detail.
  • Ability to work independently as well as part of a team.
Requirements
  • Experience: 2 years of LPN experience in a clinic setting.
  • Experience with Patients: Experience with patients of all ages is required.
  • Travel Experience: Previous travel experience is preferred.
  • Certifications:
    • Valid LPN license for New Hampshire.
    • BLS Certification (Basic Life Support) - Must be current.
  • Transportation: Must have reliable transportation as the facility is located in an area without Uber or public transportation options.
