Job Title: Community Connector
Location: Pembroke, MA
Salary: $18.00/hour
What You'll Do
Perform a variety of duties to support employment, person centered planning, self-determination and individual choice that shape the diversification and innovation of our integrated, community based services. Provide community based opportunities for skill development and learning geared toward employment and individual goals and interests. Initiate relationships within the community to assist individuals in becoming active, productive members of their communities. Provide 1:1 and group job coaching to assure that individuals have opportunities to work in jobs that meet their interests and skills. Collaborate efforts with community resources to provide quality, flexibility and enriching opportunities that meet individual interests and needs.
Responsibilities
  • Assure individuals have opportunities for enrichment, learning and skill development daily within the program and community offering daily opportunities for individuals to connect with people outside of the program and access the community around them.
  • Consistently acts with the utmost respect for human dignity and human rights, safeguarding the confidentiality of program participants and advocating with individuals for the advancement of their rights and recognition within the community.
  • Provide direct daily shift coverage in program, maintaining required ratios, following scheduled assignments away from the facilities of NEV and implementing program routines to assist participants in connecting effectively with community services and resources.
  • Develop community connections that result in supportive relationships as well as enriching employment and enrichment activities.
  • Work collaboratively with area community organizations, employers and service professionals.
  • Participate in the Person Centered Planning process for assigned individuals. Work with the individual and their employment specialist and program manager to assess individual interests and needs.
  • Work with program management to develop and teach curriculum based on age appropriate activities, ongoing learning, healthy lifestyles and career exploration.
  • Help program participants develop new skills (career exploration, private transportation, health and wellness, life skills i.e. cooking).
  • Provide consistent and accurate implementation of individual and program protocols, completing required data collection in a timely fashion. Use information to track progress and continually build skills.
  • Assure individuals have access to training and volunteer opportunities that meet their training needs and interests, providing the necessary accommodations within the community.
  • Actively participate in staff meeting, supervision and team communication using formal and informal means.
  • Provide professional, written communication through daily/weekly reports, data sheets and incident reports as required. Completes reports detailing activities within required time-frames.
  • Provide critical information for the development of the ISP, implements support strategies and attends team meetings as requested.
  • Contribute to the team, maintaining the highest levels of ethical behavior, adhering to assigned roles and responsibilities, program structure, protocols and decision-making guidelines.

Requirements
1. Must have driver's license, positive driving record, ability to pass defensive driving course and access to own vehicle during work hours (mileage reimbursement applies).
2. Strong communication skills, verbal and written.
3. Strong teaching and organizational skills.
4. Must be able to become and maintain certifications in First Aid, CPR, QBS Safety-Care, and MAP (Medication Administration Program).
5. Ability to research and access community resources.
6. Ability to represent program participants and program services in a professional manner.
7. Ability to travel extensively in the community using own vehicle and to teach others to use public transportation (when available and deemed appropriate).
8. Familiar with disability-related issues, services/resources, accommodations and assistive technology.
9. Proficient with the computer - word, excel, internet.
10. Maintain flexible schedule including occasional weekend or evening shifts.
