Liberty House, a program of Catholic Charities New Hampshire, is currently seeking a dynamic individual to join our team as a full-time Aftercare Specialist.

The Aftercare Specialist will assist Liberty House clients in their transition from residential living to a new life out in the community by helping them achieve self-sufficiency through coaching, resource referrals and connection. 

At Liberty House, our mission is to remember, honor, and support our veterans by connecting them to available resources and providing a safe, substance-free transitional house. Join our dedicated team of professionals as we assist our resident and community veterans in achieving self-sufficiency.

Duties

· Assist residents, post-separation, in achieving self-sufficiency through coaching, resource referrals and a connection back to the team at Liberty House

· Counsel former residents in overcoming dependencies, adjusting to life in the larger community, maintaining positive changes and building life skills

· Manage an active caseload of clients and provide coaching opportunities to better equip them with the skills/referrals needed to succeed

· Build positive relationships with our clients, volunteers, donors and other stakeholders

Requirements

· Bachelor’s degree in Social Services, Psychology or related field required, Master’s degree preferred.

· One to two years of experience providing direct services in a similar environment.

· Valid Driver’s License.

· Proficient with MS Office Suite 

· Working knowledge of alcohol/drug addiction and issues specific to the homeless veteran population such as PTSD.
