### Your Role:

- **MDS Coordination**: Coordinate and complete MDS assessments and care plans for our residents.

- **Accuracy & Compliance**: Ensure the accuracy and completeness of MDS assessments and care plans, adhering to state and federal regulations.

- **Team Collaboration**: Work closely with the interdisciplinary team to develop and implement personalized care plans.

- **Resident Progress**: Monitor and evaluate resident progress, making adjustments to care plans as needed to ensure optimal outcomes.

- **Assessment Coordination**: Complete and coordinate MDS assessments and resident care plans.

- **Quality Assurance**: Ensure all MDS assessments and care plans are accurate and thorough.

- **Ongoing Monitoring**: Track and assess resident progress, updating care plans as required.

· Registered Nurse (RN) with a valid New Hampshire license

· RAC-CT certification preferred

· Minimum 2 years’ experience in long term care, including experience with the RAI process and person-centered care planning

· Must have strong clinical and communication skills

· Extensive knowledge of the RAI process and reimbursement requirements for both federal and state
