Job Description:

New Leaf Brokerage is seeking compassionate and dedicated Community Habilitation Specialists to support our participants. Our specialists provide essential support and assistance to individuals with developmental disabilities, helping them to achieve greater independence and enhance their quality of life. Key Responsibilities:

• Assist individuals with developmental disabilities in daily living activities, including personal care, meal preparation, and household chores.
• Implement individualized support plans to promote skill development, community integration, and personal growth.
• Accompany individuals to community activities, appointments, and social events.
• Provide transportation and support during community outings.
• Encourage and support individuals in developing social skills and building relationships.
• Maintain accurate and timely documentation of services provided.
• Communicate effectively with individuals, families, and other team members.
• Ensure the safety and well-being of individuals at all times.

Qualifications:

• High school diploma or equivalent; additional education or training in human services or a related field is a plus.
• Prior experience working with individuals with developmental disabilities preferred but not required.
• Strong interpersonal and communication skills.
• Patience, empathy, and a positive attitude.
• Ability to work independently and as part of a team.
• Valid driver's license and reliable transportation.

• Southampton - $24, 20 hours/week
• Northport - $25, 30 hours/week
• East Hampton - $24, 25 hours/week
• Holtsville - $24, 35 hours/week
• Selden - $24, 20 hours/week
• Coram - $24, 30 hours/week
• Great Neck - $19, 7 hours/week and $22, 5 hours/week - same participant, hours will increase to 30/week over time (transitioning out current staff)
• Brentwood - $20/$25, 15/40 hours/week - two staff needed, will split hours
• Queens - $25, 40 hours/week
