New Narrative is Hiring a Community Case Manager (27848) Near Portland, OR

DescriptionShift: Sunday - Wednesday 3pm - 11pm
Purpose: (General description and summary)
The Community Case Manager is a member of the Motel Support Team: Cultivating Community that provide outreach and supplemental support to participants who are living in emergency motel placements in building skills and resource connections needed to move toward their next housing step. The Community Case Manager will provide services which include: 1:1 skills training, crisis prevention and de-escalation, community navigation and integration, case management and advocacy, coordination of transportation (ie. Ride to Care, Trimet LIFT, staff transport, etc.), collaborative meetings with treatment teams and other external providers, and referring to community based mental health and substance use disorder providers if needed. Through an integrated lens, the program aims to minimize participant hospitalizations (emergency department and psychiatric admissions), increase contact between the participant and their respective mental health, substance use service providers, or other providers/supports involved in care, and aid linkage to transitional or more permanent housing. This job requires to be on site to provide services.
Accountabilities: (Responsibilities of the job)
Supportive Skills Training
  • Providing direct support in one-on-one settings within the assigned motel and community based skills training.
  • Provide group skills training opportunities and facilitation of opportunities.
  • Assess the functional level and program needs of each participant, providing skills training as indicated.
  • Provide individual and group skills training opportunities at motel as well as in the community, including but not limited to: ADLs, iADLs, coping, regulation, safety, interpersonal, community integration, community navigation, etc. to assist in movement toward identified goals.
  • Provide support in milieu setting as indicated and opportunities arise.
Engagement and Collaboration
  • Provide skills training, advocacy, case management, community navigation to support caseload participants in working towards their identified goals.
  • Willing to meet and engage with people “where they are,” including their environment and readiness for change.
  • Meet consistently with participants within the motel setting and community
  • Providing opportunities for group skills training through opportunities for socialization, and community integration.
  • Maintain professionalism in encounters with participants and other providers.
  • Must be able to assist in de-escalation and crisis as needed
Case Management
  • Ability to work in collaboration with emergency services (ie. police, Project Respond, Multnomah County Crisis, Behavioral Health Unit) as needed.
  • Ability to work in collaboration with acute care settings if necessary.
  • Provide support, assistance, and advocacy for linkage to resources and maintenance of resources.
  • Coordinate with providers and resources involved in participant goals and care consistently, including but not limited to weekly contact and care conferences as indicated.
Crisis Prevention/De-escalation/Harm Reduction
  • Use crisis prevention approach to decrease use of crisis services, Project Respond, EMS, police, hospitalization, etc.
  • Ability to work in collaboration with emergency services (ie. police, Project Respond, Multnomah County Crisis, Behavioral Health Unit)
  • Must be able to assist in de-escalation, crisis prevention in community based settings.
  • Support harm reduction approach, linkage to resources such as, but not limited to: clean needle exchange, NARCAN, MAT, SUD services, education around harm reduction, etc.
Documentation:
  • Complete documentation and data entry as required by contract and program needs (ie. Electronic Health Record, trackers, intake, discharge, etc.) within 48 hrs.
  • Utilize centralized scheduling to account for participant appointments, meetings, training, etc.
  • Utilize outcomes based care tools in order to provide outcome informed care.
  • Ensure timely completion of all required paperwork.
  • Maintain a regular work schedule, including some evenings, weekends, and holiday depending on schedule.
  • Work to increase level of cultural sensitivity, awareness and competency regarding Person-Centered Services.
  • Attend and positively participate in required team, agency, and interdisciplinary meetings.
  • Use trauma-informed, person centered approach and language and participate in ongoing training and education.
Description of other duties:
  • Perform other duties as assigned by supervisor.
  • Provide support to teammates as indicated, ie. Milieu support, front desk support, environmental safety, etc.
  • Participate in on-call after hours phone rotation.
  • Complete all other assignments in a timely manner as requested by supervisor.

Supervision Received:
  • Receive regular supervision from the Motel Program Manager, seeking additional supervision whenever necessary.
  • Participate in interdisciplinary team meetings and group supervision as required.

  • Pursue ongoing development of skills and competencies, including:
  • A minimum of 12 hours per year continuing education in the area of mental health/dual recovery;
  • 4 hours annual training in cultural competence;
  • Follow all New Narrative policies, procedures, and protocols.
  • Follow through on all directives and instructions.
  • Maintain CPR and First Aid Certificate.
  • Must be able to pass a criminal history check through the Department of Human Services.
Qualifications:
  • QMHA not required, but preferred
  • CADC preferred, but not required
  • Minimum two years of experience working with high acuity population
  • Minimum two years working in community based settings preferred
  • Incumbents with a substance use disorder must be sober and free of substance use for one year preceding date of hire, and must maintain this sobriety.
  • Must maintain current Oregon driver’s license and insurance.
  • Must have a high school diploma or equivalent.
