New Oakland Family Centers is Hiring a Case Manager Near Rochester, MI

Job Description

Job Description
New Oakland Family Centers provides comprehensive behavioral health and substance abuse services for consumers across Michigan, in both outpatient and partial hospitalization settings.

Location: Clinton Township (primary location) with frequent travel to the Center Line and Warren NOFC locations and within the community as needed.

Schedule: Monday-Friday 8:00 am – 4:30 pm or other full-time equivalent.

Reports To: President / Clinical Director / Clinical Supervisor

Compensation: Salaried Employee; $40,000-$50,000/year. Commensurate with experience.

Position Summary: The Case Manager position is a full-time position comprised of multiple clinical and administrative responsibilities related to assisting consumers in designing and implementing strategies to obtain services and support. This position will require working closely with New Oakland therapists, MCCMH consumers, clinical leadership, psychiatrists, and community support.

Essential Duties / Functions:

  • Provides services for consumers, which may include, but are not limited to, assessments, planning, advocacy, coordination, and monitoring of access to health and dental services, financial assistance, housing, employment, education, social services, and other services developed through the person-centered planning process.
  • Ensures that the person-centered planning process occurs and results in an individual plan of service.
  • Completes documentation as required by the clinic’s policies and procedures.
  • Coordinates the consumer’s services and support with all providers, makes referrals, advocates for the consumer, and identifies and addresses any gaps in supervision.
  • Assists the consumer in accessing programs that provide financial, medical, and other assistance along with facilitates crisis planning.
  • Coordinates and assists the consumer in crisis intervention, including utilizing emergency after-hours contact, discharge planning, and assisting with the transition process, including arrangements for follow-up services.
  • Maintains accurate documentation related to mileage and consumer contacts and functions in a self-directed manner.
  • Participates in community events that provide support to our consumers.
  • Actively participates as a positive member of the clinic environment.
  • Fulfills all initial and ongoing training requirements.
  • Performs other duties as assigned.

Qualifications: Bachelor’s degree in social work, psychology, counseling, or human services related field with 1-2 years of experience in behavioral health and case management is required. Must be able to work well with all types of people and all populations including children, adolescents, and adults. An approved driving record and a personal vehicle with current auto insurance are required for travel to group homes and hospitals.

Working Conditions: Staff are to dress professionally at all times per the Employee Handbook. Staff will need to be prepared to be actively involved with consumers. Staff need to be flexible, as they may be asked to assist with coverage at another site due to census population, absences, etc.
