Social Worker - OB Cohen Hospital – F/T Days

The social worker is an integral part of the Interdisciplinary team supporting patients through their hospital course to a seamless and safe discharge. The social worker specializes in working with families providing comprehensive psychosocial, mental health, substance use and safety assessments, coordinating discharge planning for this vulnerable patient population. The social worker provides supportive counseling, linkage to community agencies and support, and referrals to homecare agencies to support a safe transition from the hospital.

This is a full-time day shift, position with rotating weekend and holiday coverage.

​ Preferred Criteria:

  • Discharge Planning experience.

  • Experience working with OB and NICU populations

Required Criteria:

  • Master’s Degree in Social Work (MSW)

  • New York State License as a Licensed Masters Social Worker (LMSW) 

  • At least one year of inpatient experience 

  • Discharge planning experience

$82,500-$105,000/Annual
