Job Title: Director of Case Management

Salary: $102,000 to $135,000 annually

Schedule: Full-time, Monday to Friday, Normal Business Hours

Job Overview:

We are seeking a dynamic and experienced Director of Case Management to lead our dedicated team. This role is pivotal in managing department activities, coordinating patient care, and ensuring optimal utilization of clinical resources. You will oversee the integration of case management with other hospital functions and external agencies to enhance patient care continuity and efficiency.

Key Responsibilities:

• Direct and oversee all aspects of the Case Management department, ensuring compliance with hospital policies and external regulatory requirements.
• Develop and implement strategic plans to enhance patient care, discharge planning, and resource utilization.
• Manage departmental operations, including staffing, budgeting, and administration.
• Foster interdisciplinary collaboration to improve service delivery and patient outcomes.
• Lead quality improvement initiatives, evaluate performance metrics, and implement process enhancements.
• Maintain strong working relationships with key stakeholders such as the CMO, CNO, and other department heads.
• Ensure staff development and competence in case management practices and standards.

Qualifications:

• Minimum of 2 years of leadership experience in acute care case management.
• Bachelors degree in Nursing, Health Administration, or related field is required; Masters preferred.
• Must hold a current RN license or LCSW certification.
• A strong clinical background with at least 5-7 years of experience, preferably in complex care settings.
• Excellent communication, leadership, and organizational skills.
