Job Title: Registered Nurse (RN) - Hospice Case Manager

Job Type: Full-Time, Exempt

Shifts: 6-7 x 12-hour shifts per bi-weekly pay period, with a mix of days, nights, and/or mid-shifts. On-call responsibilities included.

Salary: $64,480 to $82,534 annually, depending on experience

Sign-On Bonus: $10,000 with a 2-year service commitment

Overview :

Responsibilities :

  • Patient and Family Care:
  • Conduct thorough patient assessments, develop and implement personalized care plans in collaboration with an interdisciplinary team.
  • Provide direct patient care including symptom management, medication administration, and crisis intervention.
  • Offer emotional support and counseling to patients and their families.
  • Clinical Coordination:
  • Coordinate with physicians, nurses, and other healthcare professionals to optimize care plans.
  • Manage the intake, admission, discharge, and follow-up processes ensuring continuity and quality of care.
  • Documentation and Compliance:
  • Maintain meticulous records of patient care, treatment plans, and progress.
  • Ensure all care is in compliance with state and federal regulations and ethical standards.
  • Professional Development:
  • Engage in ongoing education to remain at the forefront of hospice care practices.
  • Participate in team meetings, providing insights and feedback to enhance service delivery.

Minimum Requirements:

  • Education: Graduate from an accredited school of nursing.
  • Experience: Minimum one year of RN experience, preferably in hospice, home health, or palliative care.
  • Licensure: Current, unencumbered Utah or compact state RN license.
  • Certifications: Current BLS certification; additional certification in hospice and palliative care nursing preferred.
  • Transportation: Valid driver's license and reliable transportation for home visits.
