GENERAL SUMMARY
The Licensed Social Worker position is designed to assist elders, vulnerable adults and adults with disabilities to achieve their maximum level of
health and independence in the community through a holistic, team-based approach to wellness The Social Worker conducts client assessments,
oversees the development, implementation, and monitoring of the client's plan of care and coordinates with the client's care team to ensure
client is making progress towards their goals. The Social Worker also helps to build systems of support for Social Service clients through
strategic partnerships, training, and education for client's families and services providers.
PRIMARY RESPONSIBILITIES
Primary responsibilities are the basic job duties that an employee must be able to perform. This list of responsibilities is intended to be representative of the duties performed within this classification.   • Provide diagnostic evaluation, intake assessments and treatment services adults who are managing mental illness, other disabilities or have experienced elder abuse or neglect
  • Utilize evidence-based and culturally-competent practices, and engage patient's family systems to develop treatment plans for each patient based on intake evaluation, assessment and identified treatment goals.
  • Collaborate with other providers and program staff in a multi-disciplinary approach to support patient needs in a holistic manner. Provide referrals and coordination of required services to assist patients in accessing needed care.
  • Establish client's care-team and check-in regularly regarding client's progress Encourage and support client's family members to engage in helping client meet their treatment goals.
  • Hold individual and group counseling sessions to assist with symptom management, coping skills and problem-solving strategies.
  • Work with tribal programs and community groups to implement preventative and rehabilitative social services programs and activities
  • Work with staff and partners to keep crisis response procedures updated for clients managing mental health diagnoses, for elders and for clients with disabilities.
  • Design and implement outreach and education strategies to build systems of care comprised of individuals in the community and organization.
  • Participate constructively in Quality Assurance/Quality Improvement (QA/QI) activities within the Department and organization, such as peer-review, case conferencing
  • Complete all documentation including required forms, diagnostic records and progress notes in a timely manner.
  • Follow all applicable procedures outlined in Washington Administrative Code (WAC) and adhere to all state, federal, local and Tribal laws Contribute to internal policies and procedures to ensure that they are culturally relevant, meet regulatory requirements and social work principles, ethics and best practices.
  • Ensure security and confidentiality of all program data, and operate in accordance with applicable regulations of Health Insurance Portability and Accountability Act (HIPAA) and 42CFR Part 2.
  • Use an electronic health record system for documentation, as needed
  • Other duties as assigned.
MINIMUM QUALIFICATIONS
Education and Experience
  • Masters' degree in a Social Work.
  • Active WA State licensure or Agency Affiliated Counselor Registration
  • Minimum of Two (2) years of full-time post-Masters' degree direct social work experience.
  • Excellent writing and computer skills required.
  • Must have a valid Washington State Driver's License and be insurable under the Tribe's policy.
  • Must successfully complete a pre-employment drug screen lest, a background investigation prior to employment
PREFERRED QUALIFICATIONS
  • Current on TB testing, and CPR/First Aid certified on date of hire. Experience working with Tribes or Native Communities.
SUPERVISION
  • This position supervises the Case Aid/Outreach

  • Become knowledgeable regarding resources that are available when families require additional services and/or a higher level of care than provided.
  • Work empathically and effectively with individuals of various socioeconomic backgrounds.
  • Complete multiple tasks with varying timelines in a professional manner. Follow the Mission and Vision of the program(s) and the Tribe Vision for the People of the Nisqually Tribe and Its community.
