Position: Home Health Nurse (LPN/RN)

Schedule: Saturday and Sunday 7a-7p (every other weekend will be considered)

Job Description:

We are currently seeking dedicated and compassionate Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our home health team. The successful candidates will be responsible for providing high-quality nursing care to pediatric patients in their homes, ensuring their health and well-being.

Key Responsibilities:

In-Home Pediatric Care: Provide specialized, one-on-one care to a pediatric patient for a total of 12 hours.

Follow Physician Orders: Evaluate and care for the patient according to physician's orders and advise the physician of any changes in the patient's condition or needs.

Preventative Practices: Maintain appropriate preventative practices and procedures.

Documentation: Prepare clinical and progress notes for individual patients, including physical and mental conditions as well as signs and symptoms indicative of changes in condition.

Communication: Must be dependable and communicate effectively with all members of the interdisciplinary team, the client and the family members.

License:

· Current and active FL or Multistate RN license (Required)

· BLS/CPR card via American Red Cross or American Heart Association (Required)

Experience:

· Pediatric experience (Preferred)

· G-tube, Trach and Vent experience (nice to have)

· At least one year home health experience

Credentials

· Credentialing Documents to be presented at time of application: Resume, Professional License, Driver’s License, Social Security Card, CPR Card, Physical (within the last 6 months), TB Test (within the last year), COVID Vaccination or Exemption Letter, All Applicable CEU’s

Schedule:

· Weekends only -can work every other weekend 7a-7p

Location:

· Temple Terrace FL

Pay Rate

· LPN $23-$28/hr. RN $29-$32/hr.
