JOB TITLE: IN-HOME NUTRITION AIDE

DEPARTMENT: SENIOR LIFE SERVICES PROGRAM

GENERAL DESCRIPTION OF POSITION:

The person hired into this position will be responsible for operating a county-wide Home Delivered Meals Program and Senior Center. Duties will include all aspects of operation of the Senior Center and Home Delivered Meals Programs ensuring the guidelines established by Federal, State, and the Area Agency on Aging are followed. Duties will include the assurance that clients are knowledgeable regarding existing social services available in the area. This person’s immediate supervisor is the Site Coordinator.

JOB DUTIES:

1. Comply with instructions issued by the immediate supervisor who is the Senior Center Director/Site Coordinator. Keep the supervisor informed of all aspects of Senior Center and Home Delivered Meals (HDM) operations.

2. Perform all tasks necessary for the successful operation of the HDM Program for the homebound elderly.

3. Conduct an concentrated outreach effort to locate potential participants. A portion of this effort will include referrals from other agencies and individuals.

4. Provide meals daily (five days a week) to all active program clients.

5. Provide participants (clients) with access to social outlets to combat isolation, through the use of planned friendly visits by volunteers and/or staff.

6. Provide a comprehensive information and referral service for program participants to ensure that all appropriate potential social services are made available.

7. Provide required transportation to participants when funds are available.

8. Certify that all participants in the program meet the necessary guidelines and requirements.

9. Maintain an adequate filing and information retrieval system for the HDM Program.

10. Complete all required reports.

11. Provide quality supervision to any staff and/or volunteer assigned to this program.

12. Ensure compliance with all policy and procedures from the State, or Area Agency on Aging, including eligibility requirements.

13. Perform other related duties assigned by the Site Coordinator.

DESIRABLE QUALIFICATIONS:

1. General knowledge of the problems facing the homebound elderly and the ability to deal with these problems.

2. Ability to relate to and effectively communicate with persons sixty and over.

3. Ability to effectively supervise volunteers and/or other staff assigned to the project.

4. Knowledge of existing social services and community groups concerned over the plight of the elderly.

5. Ability to keep accurate and current records on all project activities.

6. Must have the ability to drive, must possess a valid Georgia Driver’s License, have ownership or access to a reliable, safe automobile, and have adequate auto insurance coverage.

7. Self-motivation and creativity.

8. Good health to meet the demands of the position.

EDUCATION AND EXPERIENCE:

High school diploma or the equivalent. Experience and/or training in elderly services delivery. Previous supervisory experience. Additional education my be substituted for a lack of experience of training in elderly services.

TRANSPORTATION FACILITIES:

Must have access to an automobile with necessary liability insurance and be willing to use this automobile as required in the performance of job responsibilities.
