Position Summary:
North Kern South Tulare Hospital District (“NKSTHD”) is looking for a Social Services Assistant to join our team at our Delano District Skilled Nursing Facility (“DDSNF”), located in Delano, CA. The primary purpose of your job position is to assist the Director of Social Services in planning, organizing, developing, and directing the day-to-day functions of the Social Service Department. As a social services assistant, you provide vital support to social workers and other mental health professionals in offering services to the community that they desperately need. You may deal with rehabilitation; psychological and social work needs as you provide support for residents and their families. As a social services assistant, you can change lives by providing aid many don’t even realize is available to them. Your goal is to identify needs and determine the best ways to overcome them with the services available to the citizens of the community.
Essential Functions:
· Assist the Social Service Director in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility.
· Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed.
· Assist in the development, administering, and coordinating of department policies and procedures.
· Assist in meeting residents’ ancillary service’s needs.
· Maintain concern grievances and theft and loss logs.
· Participate in meetings to ensure that the facility is meeting needs of the resident and family (i.e.: Care Conferences)
· Participate in discharge planning, development and implementation of social care plans and resident assessments.
· Provide information to residents/families as to Medicare/Medicaid, and other financial assistance programs available to the resident.
· Record and maintain regular Social Service progress notes as needed or when directed by supervisor.
· Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
· Compile and maintain a social service directory listing available community resources.
· Compile information on discharge plans and present it to supervisor for effective discharge planning.
· Ensure that donated or unlabeled clothing is labeled, organized and up to date.
· Work with the facility’s consultants as necessary and implement recommended changes as required.
· Make routine visits to residents and perform services as necessary.
· Make written and oral reports/recommendations to the supervisor concerning the operation of the social service department.
· Assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.
· Participate in completing appropriate sections of the MDS as necessary.
· Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the supervisor.
· Performs other duties as assigned.
Qualifications:
· Ability to work with a team and communicate professionally with clients and colleagues.
· Excellent organizational skills and attention to detail
· Excellent communication, written and verbal.
· Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the public.
· Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
· Must be able to relate information concerning a resident's condition.
· Must not pose a direct threat to the health or safety of other individuals in the workplace.
Schedule:
Monday through Friday 8a-4:30p
Some Weekends If Needed
Some Overtime may be required.
