Therapeutic Case Manager, (Concord)

You'll have the opportunity to work closely with school personnel to craft comprehensive treatment and transitional plans for students across Caledonia County. Schedule follows school calendar and includes five week summer program. 40 hours/week, 7:45 am - 3:45 pm.

Responsibilities: Therapeutic Case Manager
- Maintain all clinical records in compliance with Clinic/Medicaid requirements for therapeutic case management.
- Attend meetings and accept duties at the request of the Program Manager.
- Provide casework services including, but not limited to:
- Development and Coordination of Individual Plan of Care;
- Collaborative teaming with child, family, school, and community resources;
- Provide and coordinate services including home-based interventions, school based services, parenting support and group work;
- Coordination of crisis and transition plans;
- Complete or arrange for a comprehensive assessment;
- Establish a trusting and helping relationship
- Education and public awareness.
- Have a vehicle available to drive, maintain a valid driver’s license, and have proof of a minimum of $300,000 liability insurance.
QUALIFICATIONS
- Bachelor’s degree in Human Services, Master’s degree preferred. Will consider candidate within 6 months of earning undergraduate degree with experience providing individualized support services to emotionally disturbed children.
- Background in programming for children on the autism spectrum preferred.
SALARY & BENEFITS
- $21.00/hr plus, depending upon education and experience
