Northeast Ohio Home Care is an expanding non-profit, skilled home health company that focuses on helping older adults maintain their independence in their own homes. A large portion of our caseload is in Assisted Living and Independent Living communities. We are looking for a home health Registered Nurse who enjoys freedom and flexibility! Schedule:

• Monday-Friday - 1 weekend a month - 1 holiday a year
• Hours: 8:00-5:00

Qualifications for Home Health Registered Nurse / RN Clinical Manager:

• RN required
• 1 year of acute care or home health experience (preferred)
• BLS CPR certification
• Active Driver’s License

Position summary for Home Health Registered Nurse / RN Clinical Manager:

• Completes in-person patient assessments to decrease the risk of re-hospitalization
• Provides direct skilled nursing care - assessments, symptom management, wound care, patient education
• Supervision of patient caseload
• Educate and advocate for patients and their families
• Coordinates care with the home health team and maintains communication with physicians
• Maintains strong patient management and documentation skills
• Communicates well with patients, families, doctors, and home health team
• Displays a comfort level with computers / technology
• Displays strong problem solving and critical thinking with their patients
• Self-directs, self-schedules, and works independently
