Northeast Valley Health Corporation is Hiring a Patient Navigator I Near Pacoima, CA

The Patient Navigator works within Northeast Valley Health Corporation to perform a variety of health education, clerical, and social support services within the clinic setting and in the community. This person has direct knowledge of the local community and functions as a “guide” and offers assistance to patients in “navigating” the health care system and accessing related social and financial services to facilitate the care of individuals and their families. The Patient Navigator will maintain communication between patients, their families, providers and care team, managed care organizations, and community organizations. The Patient Navigator will work directly with the Transitions of Care Nurse to support the patient’s transition back to Primary Care, and will assess needs related to Social Drivers of Health (SDoH), providing referrals, support and follow-up to ensure care coordination.
Reports to: Associate Director of Health Equity
Qualifications:
1.High school diploma or General Educational Development (GED) certificate.
2.Good written and verbal communication skills.
3.Ability to be trained to perform a variety of tasks.
4.Must have direct knowledge of the local community.
5.Strong interest in the community and its people
6.Committed to ongoing learning, able to model the behavior and activities that will support both individual development and community capacity-building.
7.Possess communication and other interpersonal skills that can ensure an ability to work with local people and organizations.
8.Sensitivity to the different cultures represented among members and staff.
9.Demonstrated ability to listen and communicate with others in a professional and caring manner including sensitivity with individuals from diverse cultures and lifestyles.
10.Fluency in English (speaks, read and write).
11.Fluency in Spanish (speaks, translate, read and write).
12.Ability to maintain absolute confidentiality about health care and other patient/client information.
13.Current California driver’s license, appropriate insurance coverage and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).
