The Community Health Worker is a full time position that will provide confidential and compassionate support to connect community residents with resources available to them. This position will support the communities within Danville Health Center and St.Johnsbury Community Health Center. Collaborating with local partner organizations is essential to this position. Responsibilities include but are not limited to providing application assistance for State and Federal Programs, health insurance guidance and enrollment, assistance with accessibility to food, fuel and transportation solutions. A reliable vehicle is a must as travel between our health and dental sites will be required.
