We have big game hunting of deer, elk, sheep, bear, moose, and mountain lion; fishing on a myriad of mountain lakes, streams and on the Kootenai River; and year-round outdoor recreation for the avid or beginning recreationalists – including skiing, hiking and mountain biking. All of this with a backdrop of the Cabinet Mountains wilderness, which towers 9,000 feet from the valley floor.

Patient Care Coordinator
DESCRIPTION
NWCHC is looking for a full-time Patient Care Coordinator to efficiently mange internal and external patient referral coordination, conduct population health management, and provide basic care management duties. The position is responsible to coordinate the scheduling and follow up of specialty care referrals, deliver necessary document to the referral entity and ensure effective coordination with health center patients. This position is also responsible to effectively manage verbal and written patient communication; perform outreach through patient follow up and reminder calls; schedule appointments; obtaining and organizing patient information; inputting patient, medical or demographic information into an electronic patient record and perform other administrative tasks as assigned. The ability to exercise initiative, sound judgement, strict confidentiality and cooperation is essential. Wage will be dependent upon qualifications and experience.
