Description

The Social Work Clinical Coordinator is a Licensed Clinical Social Worker who reflects the mission, vision, and values of Northwestern Memorial, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

The Social Work Clinical Coordinator is an experienced Social Work Practitioner, who in addition to providing direct patient care, is accountable for facilitating staff assignments, coordination and monitoring of workload and is accountable for ensuring the delivery of quality patient care and patient throughput during the assigned shift.

Responsibilities:

  • Provides technical/clinical direction and guidance to staff to insure standards of high quality are achieved and maintained in the delivery of patient care and compliance with policies, procedures and clinical protocols.
  • Facilitates department operations according to policy, procedure, and regulatory requirements.
  • Coordinates department staffing assignments to achieve high quality and cost effective care.
  • Directs patient care provided by staff social workers.
  • Communicates directly with patients and significant others about social work care.
  • Implements appropriate action plans to provide safe, quality, and efficient care.
  • Analyzes patterns of daily activities and recommends operational/program changes as needed.
  • Anticipates staffing needs for subsequent shifts and makes necessary arrangements for coverage.
  • Facilitates effective interdepartmental communication of incidents with appropriate personnel (manager, administrator, medical staff, etc.). Identifies and communicates key issues and trends to managers and makes recommendations for improvement on an ongoing basis.
  • Provides input to manager on budgetary issues and cost containment strategies.
  • Establishes relationships across departments to optimize patient care.
  • Assesses and communicates staff development needs to manager.
  • Orients staff to Clinical Coordinator role.
  • Serves as a resource for staff.
  • Participates in Transition into Practice through managing new hire assignments and mentoring.
  • Responds to urgent issues such as disaster alerts, emergencies, and employee injury appropriately and in a timely manner.
  • Participates in interviewing and hiring process.
  • Integrates theoretical knowledge, evidence based principles, and clinical expertise into daily social Work practice and educational experiences. 
  • Participates in formal and informal education of the staff, including Social Workers, Social Work Coordinators, and students including weekdays and weekends.
  • Reviews and provides input on all departmental and Social Work related policies, procedures and protocols.
  • Collaborates on developmental plans for individual staff.
  • Provides direct patient care as a portion of their assignment.
  • Demonstrates effective leadership and problem solving skills.
  • Supports collaborative education and team building.
  • Participates in multidisciplinary quality initiatives to enhance clinical care and patient outcomes.
  • Participates in research and uses established research critique guidelines when reviewing evidence-based articles.
  • Assists manager with continuous review and updating of department reports.
  • Understand the CMS and TJC standards as they relate to direct patient care.
  • Provides feedback to the manager regarding staff performance and assists in completion of the Performance Management Process (PMP) for social workers and support staff.
  • In absence of the manager, effectively assume his/her responsibilities to carry through the operations of the department
  • Serves as on-call leadership support on a rotating basis.

Additional Functions:

  • Champions new initiatives and participates in unit/departmental/divisional or hospital committees/task forces as assigned.
  • Remains clinically proficient within area of expertise.

Qualifications

Required:

  • State of Illinois licensure: Licensed Clinical Social Worker (LCSW)
  • Master’s Degree in Social Work from a School accredited by CSWE
  • Minimum of five years post Master’s experience in a health care environment or related setting
  • Certification or advanced training in area of specialty
  • Demonstrates alignment with NMH Core Values of integrity, excellence, patients first, and teamwork
  • Effective communication and interpersonal skills
  • Ability to plan, organize and supervise the work of other Social Workers
  • Provided a minimum of 4 semesters of graduate field work instruction

Preferred:

  • Member of the NASW, or other relevant Social Work professional organization
  • One year employment at NMH with a PMP of outstanding performer or role model
  • At least 2 years of leadership experience in healthcare or related setting
  • Demonstrated project management experience
