Working under routine supervision, performs various patient care activities in meeting personal needs of, pediatric, adult and geriatric Home Health and Hospice patients in their homes and assists patients and families in achieving safe and sanitary physical comfort. Services that are provided are ordered by the physician, included in the plan of care, permitted under state law, and are consistent with the aides training. Duties include but are not limited to:

Home Health Aide registered in the state of Indiana with hospice competency testing; previous Home Health and Hospice experience preferred; continuing employment is contingent upon maintaining 12 hours of annual continuing education.
