The RN Case Manager works under minimal supervision performing a broad range of duties,
Completes screening assessments from admission through discharge to identify patient needs in a timely manner;
Taking patient and caregiver preferences into account, coordinates services to maximize patient outcomes includes but is not limited to non-acute care referrals and placement, Home Health, Hospice, Durable Medical Equipment;
Utilize evidence based care guidelines to ensure appropriate level of care and length of stay
Work as a part of a multi-disciplinary team to promote effective, efficient, and timely care resulting in positive outcomes for patients;
Aids patient and families in the engagement of resources while taking into consideration financial, social and clinical factors;
Works closely with social services on more complex psychosocial and special needs;
Completes quality audits as requested;
Participates in quality improvement initiatives
