Novel Home Health is an established yet growing Home Health agency. We are increasing our field team so we are looking for nurses who are able to provide an outstanding level of care.

We have multiple positions open in Dallas, Fort Worth and all surrounding areas.

RNs would be responsible for:

• Completing comprehensive assessments, including completion of Oasis and other documents.
• Perform skilled nursing visits as needed.
• Coordination of care with the LVNs, HHAs, Clinical Manager and therapy staff.
• Coordination of care with physician offices
• Supervisory visits as needed.

All nurses would be required:

• Attend In-service meetings and case conferences.
• Provide communication via phone and patient comm notes in Kinnser regarding patient issues
• Timely Documentation

Bilingual (Spanish/English) a plus!

Knowledge of Kinnser/Wellsky software required.

Hospice: knowledge of Healthcare First is a plus!'
