NTT DATA Services currently seeks a Care Management Associate-Discharge Planner to join our team for a work from home opportunity in Phoenix, AZ (must reside in Maricopa county). This is a 1 year contract role. Will be required to come onsite 1st day and occasionally through the project.

Utilizes critical thinking and judgment to collaborate and inform the case management/concurrent review process, in order to facilitate appropriate healthcare outcomes for members by providing care coordination, support and education for members through the use of care management/concurrent review tools and resources. Will assist with discharge plans, and outreach to members, assist with HH/DME issues.

Role Responsibilities

-Coordinates care with providers as a part of comprehensive and proactive discharge planning.
-Identifies and escalates quality of care issues through established channels.
-Utilizes negotiation skills to secure appropriate options and services necessary to meet the member’s benefits and/or healthcare needs.
-Utilizes influencing/ motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health.
-Provides coaching, information and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices.
-Helps member actively and knowledgably participate with their provider in healthcare decision-making.

Required Skills/Experience

* 2 years experience discharge planning experience

* 2 years customer experience or call center experience in healthcare

* 1 years working with multiple computer systems with a strong computer literacy and the ability to navigate through internal and external systems

* 1 years medical terminology and concepts of care management

* Typing assessment 45wpm or higher

Preferences

-Effective communication, telephonic and organizational skills
-Must possess strong customer service skills to coordinate services

Education: verifiable high school diploma or GED

Hours: 8am-5pm-may vary
