The Nueces Center for Mental Health and Intellectual Disabilities (MHID) is dedicated to helping the residents of Nueces County who are living with mental illness and/or intellectual and developmental disabilities. We serve over 10,000 people each year with psychiatric and behavioral health care outpatient services, counseling, training, employment assistance, housing support, homeless outreach, children's mental health care and more.

Salary: "Please note that the job posting rate for this position is $20.85 to $23.12, the salary offered will follow the starting salary guidelines which are typically made at or near the minimum of the salary range. However, a salary may be negotiated above the minimum range depending on applicable years of experience and qualifications."

Full-Time, 40 Hour Work Week, Paid Bi-weekly

General Description
PATHcaseworker will serve as a liaison between homeless individuals and mainstreamcommunity service system. Services willbe mobile in nature contacting homeless persons in the community to identifyservice needs to reduce homelessness by linkage to appropriate agencies withinthe community. Ongoing services will beprovided to the homeless population identified as having a mental illness. Understandsand implements trauma informed care practices. Responsible for delivery of casemanagement. Identifies clients' rehabilitation needs and provides the necessaryinstruction in teaching self-help, self-care, or other skills according to theneeds outline in client's recovery plan. Crisis prevention and interventionwill be carried out. Services will be provided in the community with the goalto successfully mainstream this population into community services. Individual improvement of homelessindividuals is based on the following functioning scales: decrease the numbersof incarcerations; stability of mental health functioning; decrease housinginstability; decrease of substance use; and engage individuals and link toservices to improve perception of recovery. Works under PATH supervisor withlimited supervision and considerable latitude for the use of initiative andindependent judgment.

Minimum QualificationsBachelor's or advance degree from an accredited college or universitywith a major in psychology, social work, medicine, nursing, rehabilitation,counseling, sociology, human growth anddevelopment, physician assistant, gerontology, special education, educationalpsychology, early childhood education or early childhood intervention. Knowledge of community resources of case managementprinciples, objectives, standards, and methods of program policies andprocedures. Ability to assess client needs, coordinate client services andcommunicate effectively. Must have a Valid Texas Driver's license with a gooddriving record and reliable transportation. Preferred: One year of workexperience in social services. Bilingual in English and Spanish.
