The ideal candidate will have a strong background in patient care and medical records management, with a focus on providing exceptional primary care services.

Requirements:

• Prior experience in primary care or related healthcare fields
• Proficiency in medical records management and patient service coordination
• Strong knowledge of medical terminology and infectious disease care
• Ability to provide compassionate patient care in various medical settings
• Excellent communication skills and the ability to work effectively in a team environment

Duties:

• Coordinate patient services and ensure the delivery of high-quality medical care
• Manage medical records and documentation efficiently
• Provide direct patient care, including medical-surgical and infectious disease care
• Demonstrate a deep understanding of physiology, medical terminology, and critical care practices
• Collaborate with healthcare professionals to optimize patient outcomes
• Conduct comprehensive initial assessments and develop personalized care plans for assigned members.
• Establish and maintain strong rapport with members, managing a caseload of up to 25 individuals.
• Assist members in accessing community-based resources, scheduling medical appointments, and completing necessary referrals for both community and health services.
• Document all interactions and activities accurately using the designated health portal.
• Conduct outreach tasks and conduct face-to-face visits with members in a clinical setting.
