Oak Ridge Care Center in Union Grove, WI is seeking for a Long-Term Care Social Worker to join the nursing team.

The Social Worker will assist the physician, resident, resident’s family and the facility’s staff in identifying the needs and problems of the resident. To assist the resident in adjusting to his/her illness and to the living in the facility as a long-term care resident. The Social Worker will accomplish this by assimilating information about the resident, planning, counseling and the appropriate use of community resources through selected referrals and coordination of services. He/she will work closely with the Admissions Coordinator/Community Liaison.

This position is a Monday-Friday

Every 4th weekend on call for new admissions

Duties and Responsibilities:

  • Counsels’ residents and families to inform them of services offered at the facility and their rights and responsibilities.
  • Provides direct services utilizing available resources in helping the resident solve problems associated with illness, disability and the aging process.
  • If necessary, makes referrals for guardianship proceedings, or to appropriate agencies in cases of financial, psychiatric, rehabilitative or social problems which the facility cannot serve.
  • Provides continuing social service assistance to and communication with the resident, guardian, family or other responsible persons and other community resources.
  • Provides assistance to other facility staff and the resident in discharge planning to ensure the safest and least restrictive environment for each resident.
  • Participates in in-service training for staff on the emotional and social problems and needs of the aged and ill and on methods for fulfilling these needs. Responsible for coordinating and conducting Abuse Prevention and Reporting training for employees who are within their 90-day probation period. Responsible for conducting a yearly Residents’ Rights/ Abuse Prevention and Reporting In-Service for all staff.
  • Obtains, records, and updates social service information:

o Social Service Discharge notes

o Social history

o Social Service progress notes (with every care plan staffing held or significant concern/change)

o Resident care plans

o MDS assessments, CAAs

  • Knowledge of outside resources and referral agencies to provide services beyond our capabilities such as, but not limited to psychiatric, legal, financial and home health care. May require driving facility van on occasion.
  • All other duties as assigned by the Administrator.

Qualifications:

  • Shall be accessible, patient, and easy to talk with individual who will provide care and concern for the resident’s psychosocial needs.
  • Genuine interest and concern for the geriatrics.
  • Familiar with community health and social resources in order to obtain services the facility does not provide and to aid in discharge planning.
  • Ability to work efficiently with little or no supervision.Ability to work and maintain open lines of communication with residents, families, staff and community resources.
  • Ability to manage complex situations professionally and timely.
  • Bachelor’s degree in Social Work or a related field is required.
