What You’ll Do as a Case Manager:
Case Managers help youth live healthier and more fulfilling lives through identifying and supporting their needs and goals. Care facilitators specialize in:
Case Managers help youth live healthier and more fulfilling lives through identifying and supporting their needs and goals. Care facilitators specialize in:
- Managing the care provided to youth in the residential setting
- Helping youth gain access to the services they need to meet their goals
- External collaboration with community partners
Primary duties as a Case Manager:
- Assessment of the eligible individual to determine service needs
- Development of a specific care/treatment plan
- Referral and related activities to advocate and assist the individual obtain needed services
- Monitoring and follow up of the plan
- Evaluation (of impact of treatment plan on recovery)
- Interact frequently with the person’s treatment team including residential therapists, physicians, other treatment providers involved, community agencies, referral sources and other services.
- Linkage and networking with DCS, court, schools, health care providers, employers and other community agencies
- Provide integration and on-going assistance to youth and families to help sustain within the community/least restrictive environment
- Accountable for the flow of information within a multidisciplinary team setting
- Special Populations/Skills/Employment duties, as assigned
- Hours may include availability in the evenings and/or weekends
Minimum Hiring Requirements for a Case Manager:
- BA/BS in social service or a human services-related field required.
- Two years’ experience in a social service setting preferred
- Must have a valid driver’s license and proof of insurance
