Job Duties and Responsibilities:

• Ensures prompt and accurate data input into Initial Contact and Intake data into agency database, CAPTAIN.Follows HUD / ESSHI / HCR / OMH regulations and/or standards. Knowledgeable in tenant’s rights and fair housingpractices.Perform on-going in-home visits to provide supportive services, this may include crisis intervention, supportive counselling,advocacy, and developing and aiding in accomplishing tenant-based goals.Completes accurate documentation and timely paperwork including but not limited to maintaining tenant files, annualadmission renewal, case notes based on frequency of visit and support provided, quarterly support planning and needassessments.Conducts strength-based assessments to assist in gauging needs, personal strengths, and goals. Assists in developing SMARTsupport plans. Monitors outcomes and updates support plans, accordingly, periodically reassesses needs.Supports tenants in attaining their goals by providing education, advocacy, and linkage. This may include referrals/linkage tocommunity services and resources to meet basic needs and obtain adequate medical, dental, and emotional health.Assists tenants with securing funding, proper clothing and ensuring adequate food supply.Provides and/or assists in securing necessary transportation to access services, accompanies tenants to appointments asnecessary.Provides assistance with tenants’ money management skills and encourages prompt payment of rent, bills and otherresponsibilities.Assists in advocating with funding resources, this may include completing shelter verifications, assisting with collectingdocumentation, submitting paperwork/ updates, providing transportation and/ or attending meetings, etc to ensure continuedservices.Works cooperatively with other agencies and community providers.Acts as a liaison between tenants and landlord, neighbors, or service providers when a communication barrier is present.Ensures that safety and sanitation is maintained in apartments and participates in necessary maintenance reviews.Provides guidance and direction to tenants in proper home upkeep, food, and medication storage, ensures laundering ofclothes/towels and bedding.Assists with education and direction to address prevention of future evictions or homeless situations.Assists with any necessary planned move-outs or evictions of tenants.Performs related duties and special projects as required; provides recreational activities as scheduled.Attends related trainings and in-services, participates in annual Point-in-Time CountShare on-call responsibility with other HOPE housing sites on a rotating basis.

Job Requirements:

Must demonstrate excellent communication, interpersonal, customer service, organizational, time management, ability towork independently, and verbal and written skills.Must have ability to relate to special populations and individuals/families with multiple life difficulties in a warm, non-judgmental manner, and be a good role model to clients.Must demonstrate an ability to problem solve, de-escalate, and provide crisis support.Must complete all job duties in accordance with a Housing First approach / philosophy, Person Centered Care, TraumaInformed Care, Harm Reduction PracticesMust demonstrate knowledge of community services and resources, awareness of location, purpose and how to access, referand link clients with services.Must demonstrate knowledge of housing regulations, ESSHI, HUD, HCR, Fair Housing and OCO Procedures and PoliciesMust be able to work a flexible schedule to meet the needs of tenants and program; must share on-call.Must be willing to actively participate in activities with tenants.Must be able to establish/maintain/exhibit professionalism with tenants, other employees, and other service providers.Must be creative in program planning, demonstrate resourcefulness, initiative, and objectivity.Must have computer proficiency in Microsoft Office and the ability to navigate the internet.Must have acceptable physical and mental health to carry out duties and responsibilities of the position: able to climb stairs,bend, lift 25 lbs, able and willing to assist with moving tenants in and out as well as routine cleaning of apartments andcommunal areas.Must possess a valid NYS Driver’s License with a good driving record within Agency policy and have access to a reliable,properly insured vehicle for travel and transport of clients.

Minimum Qualifications:• Associate degree in a Behavioral Sciences or Human Services Field, and
• One to three years related experience working with special populations, in a human services or housing program field, or
• Any acceptable combination of education, experience, and training.

Full-Time 37.5 hours per week / 52 weeks per year

Grade 9

Friday - Monday: 1:00pm to 9:00pm

Tuesday: 12:00pm - 8:00pm
