PREVENTIONIST

REPORTS TO:Administrator and Director of Nursing Services

This job description, defining qualification, primary and secondary functions, responsibilities and duties, is intended as a guideline for the employee and manager. The duties listed here are not considered to be all inclusive. The employee in this position performs these and other duties as required although not listed which may be assigned by his supervisor or facility Administrator.

ESSENTIAL FUNCTIONS

Plans, implements, directs and evaluates the education program in the facility based on identified resident needs, staff needs, federal, state and local regulations.

Evaluates resident and staff needs, and provides the Administrator and Director of Nursing Service with comprehensive recommendations concerning the educational needs of all facility personnel.

Interprets and implements established facility policies, procedures and regulations to all facility personnel.

Plans, schedules and posts a monthly calendar of education programs in a timely manner.

Prepares lesson plans containing objectives, course content, method of instruction and method of evaluation for each class scheduled and completed.

Supervises classroom and clinical components of a CNA training program.

Manages the approved CNA education program and submit approval forms as required by the state agency.

Submits, in writing, any CNA program changes to appropriate regulatory agency.

Continually assesses the CNA education program to assure that the approved program is implemented.

Observes and/or teaches the facility safety rules to all personnel.

Observes and/or teaches the facility Resident's Rights policies and procedures to all personnel.

Responsible for the planning, organizing, implementing, evaluating and coordinating a total staff educational programs.

Develops and implements written facility orientation program for all new personnel.

Coordinates and supervises the facility program of orientation with Department Heads.

Assumes responsibility for obtaining continuing education relating to the care of the geriatric resident and nursing education.

Maintains current education records of classes conducted and attendance of all personnel including CEU programs and mandatory in-services.

Works collaboratively with Department Heads to evaluate and retrain employees with problems related to job performance.

Monitors and implements infection control program in collaboration with the facility staff.

  • Familiar with NHSN,CDC, D.P.H. Board Of Health Reporting

Monitors employee health matters including coordination of worker’s compensation issues with administration/insurer.

General Requirements:

Demonstrates knowledge and understanding of the impact of disability upon an individual's, as well as families' physical, emotional, social, economic, and vocational status, taking into consideration age-related issues, and developmental levels over the life span (i.e., the adolescents' need to develop their own identity and cope with life changes; and the geriatrics' need to maintain their integrity and independence and adjust to the physiological aspects of aging), and meets competency requirements for the population served (adolescent through geriatric), as applicable to the job description.

Demonstrates consistent ability to work cooperatively with residents, supervisors, nursing staff, physicians, families, consultants, community agencies, ancillary service personnel and administration.

Comes to work as scheduled and consistently demonstrates dependability and punctuality.

Presents a professional appearance.

Assumes accountability for all data contained in the personnel policy manual.

Participates in Quarterly and Department Head meetings.

NON-ESSENTIAL FUNCTIONS

Plans and provides the Administrator and all Department Heads with an annual schedule of planned education programs.

May coordinate orientation for volunteers.

Involves staff in total program planning and implementation.

Provides and uses a variety of teaching methods, materials and resources for program presentations.

Participates in the employee evaluation process by providing training attendance records of all personnel.

Establishes a resource file for speakers, literature and community contacts.

Prerequisites:

Graduate of an accredited school of nursing.

Current registration in Massachusetts.

Infection Control Certification-C.I.C.

R.N.B.S. desirable, graduation from accredited 3 year program or R.N.A.C. acceptable.

Formal preparation and/or experience in training of adult groups preferred.

At least 3 years of nursing experience as a registered nurse.

Knowledge of Restorative Nursing Practices preferred.

Excellent written and oral communications skills required.

Knowledge of State, Federal and J.C.A.H.O. regulations preferred.

Knowledge of growth and development across the life span required.
