Ohel is seeking a Licensed Registered Nurse for a dynamic field-based care transition team. This role will ensure seamless integration into community services, providing holistic care and medical support.

Responsibilities

• Conduct holistic health assessments and maintain records.
• Collaborate with healthcare team for comprehensive care planning.
• Educate patients on health maintenance and disease prevention.
• Monitor patients' conditions and report changes.
• Provide health education to patients and families.
• Conduct counseling sessions under supervision.
• Administer emergency care and crisis intervention.

Qualifications

• New York State RN license with at least one year of clinical experience, preferably in behavioral health.
• Knowledgeable in health promotion and disease prevention, and able to recognize abnormal signs and symptoms.
• Familiar with medications and potential adverse reactions.
• Skilled in emergency treatments and interventions.

The ideal candidate is a self-starter who can work independently as well as within a team and has excellent follow up skills. This 35 hour a week, full-time position is located in Brooklyn or Queens, offers a hybrid schedule, with inpatient, home, and community visits required. Position includes a competitive benefits package.
