DescriptionA leading not-for-profit, Federally Qualified Community Health Center is seeking to add an outpatient Psychiatric Nurse Practitioner to their multi-disciplinary team. This role involves providing diagnostic interviews, medication management, and therapy to patients of all ages, with approximately 25% of patients being adolescents.
You will conduct health assessment interviews, perform complete physical assessments, and order and interpret diagnostic tests. Developing a plan of care based on diagnosis and individual client needs is essential, as is prescribing medications and treatments as appropriate. Collaboration with the healthcare team and making referrals to other disciplines will be key responsibilities.
Responsibilities
You will conduct health assessment interviews, perform complete physical assessments, and order and interpret diagnostic tests. Developing a plan of care based on diagnosis and individual client needs is essential, as is prescribing medications and treatments as appropriate. Collaboration with the healthcare team and making referrals to other disciplines will be key responsibilities.
Responsibilities
- Conduct health assessment interviews and perform complete physical assessments.
- Order and interpret diagnostic tests.
- Develop plan of care based on diagnosis and individual client needs.
- Prescribe medications and treatments as appropriate.
- Collaborate with the healthcare team and make referrals to other disciplines as indicated.
- Record, report, and interpret clients’ responses to the plan of care.
- Teach, counsel, and assist individuals and families in health promotion and illness prevention.
- Collaborate with team members in providing regular health screenings and health promotion measures.
- Identify community resources helpful to clients and assist them in utilizing such resources.
- Spanish speaking preferred, but not required.
- Flexible work schedule from Monday to Friday.
- Choice of locations includes Harlem, The Bronx, or Upstate NY.
