Overview:

FT/40hrs. Responsible for the coordination of care for clients, especially those involved with multiple systems of care. Facilitates Child and Family Team Meetings to ensure that Child, Family, and all formal and informal supports have voice in the process. Responsible for facilitating the development of the client’s integrated care plan. Qualified candidates must possess a related Bachelor’s degree and a minimum of 1 year relevant work experience or equivalent and speak fluent Spanish. Starting pay rate of $26 per hour.

Responsibilities:

• Assemble the Family Team by interviewing the youth’s family, identifying extended family supports, natural supports, community supports and formal supports, including referring party.
• Coordinate and facilitate the meeting of the participants’ Family Team based on the family member strengths and needs.
• Assist the participants and the participants’ family in understanding and fully participating in the process. Ensure that the participants and the participants’ Family Team are involved in all phases of determining the goals and needs.
• Develop, with the youth and family, an individualized Plan of Care, Family Budget, and Family Safety Plan, reflecting the youth’s and family’s culture and values.
• Assist the youth and family to access strength-based services, including mental health, education, social service and other supports as identified by the Family Team.

Qualifications:

• Must have a bachelor’s degree in a related field.
• Be 21 years or older due to County contract requirements.
• Maintain a valid California Driver’s License and acceptable driving record of at least 3 years as required by Olive Crest’s insurance carrier.
• Must maintain vehicle insurance throughout course of employment.
• Maintain criminal record clearance.
• Must have CPR and First Aid certification upon hire and maintained throughout employment.
• Must have excellent written and verbal communication skills.
• Must be able to lift and carry up to 30lbs.
• Must be able to walk on unstable ground and to stand for long periods of time.
• Must be able to bend, stoop, reach, use hands/ feel, climb, balance, and walk frequently.
• Apply reason in problem-solving situations where only limited standardization may exist. Interpret a variety of instructions furnished in written, oral, diagrammatic or schedule form.
• Must have a philosophy of support for at-risk children and families in line with the mission, vision, values, and statement of faith of Olive Crest.
