Olivia Restorative Therapy and Nursing - The MDS Coordinator is a accountable for overseeing the completion, editing, locking and transmission process of all MDS assessments. The overall responsibility of this position is to help assure timely completion of the MDS with a focus on accuracy, case mix maximization, and regulatory compliance. He / She oversees the clinical and regulatory component of MDS completion related to reimbursement. The Position also coordinates care plan completion with the interdisciplinary Team (IDT) and renders clinical support to nursing staff to enhance accurate assessments, care plan and excellent quality of care. Essential Responsibilities And DutiesResponsibility: To assure timely completion of the MDS with a focus on accuracy, case mix maximization, and regulatory compliance
Completes daily dashboard management to ensure quality care for the resident.
Completes MDS in Electronic Clinical System based on observation of the resident, record review, data collection worksheets, NAR data, and interview with residents, families, and staff.
Updates resident information in Electronic Medical Records documentation system as each resident's assessment window opens and communicates that information to all staff responsible for entering resident data into Medical Record system.
Oversees timely completion of the MDS
Completes Care Area Assessment (CAA) documentation based on regulatory requirements, MDS information obtained, and in accordance with professional standards of practice.
Works with the Interdisciplinary Team (IDT) to help assure care plans are comprehensive and completed on time.
Manages the Care Conference Process and assures timely completion and follow through.
Submits completed MDS within specified time frames; monitors validation reports for errors or problems; corrects errors or problems, as needed.
Assists the Interdisciplinary Team (IDT) and oversees completion of necessary paperwork including Medicare Certification and recertification forms; Prior Authorization; required notices; demand bills (including required paperwork for medical assistance payor); clinical documentation to support Medicare coverage; Monitors for Medicare eligibility after resident is admitted, e.g., rehospitalization or 30-day window.
Oversees eligibility for payors such as MSHO or insurance, and completion of necessary paperwork to assure coverage.
Provides incidental direct care several times a week to monitor residents' needs and staff interactions with the goal of increasing MDS coding accuracy.
Provides nursing care in emergency situations and as directed by the Director of Nursing.
Take on call rotation as directed by the Director of Nursing.
Assist with training of staff, as needed, to help improve clinical care and accuracy of MDS Coding
Participates in Abuse Neglect Investigations as needed

Provides Rehabilitation Center with current license and keeps updates
Attends continuing education/In-service pertinent to your department and designated by the DNS
Attends nurse meeting at the Rehabilitation Center
Attends all staff education on the Rehabilitation Center relating to safety, infection control and disease process.
Attends educational programs on Continuous Quality Improvement, customer satisfaction and other programs. Participates in inspection and audits as designated
Maintains current Knowledge of Minnesota Department of Health Regulations.
Maintains and applies knowledge of current policies and procedures as outlined.
QualificationsEducation and Experience
Must be a Registered nurse (RN) in the state of Minnesota
Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for health care provider.
Must possess the ability to communicate verbally and in writing when directing the Resident Assessment Instrument (RAI) process and when performing and assessing care for residents.
Must be able to perform essential functions of the position with or without reasonable accommodation.
Must qualify for employment after criminal background study, per guidelines of Minnesota Department of Human Services.
Other Duties as assigned
Preferred
Experience in healthcare, skilled nursing facility or assisted living community
