Preceptorship semester and application deadline:

• January - April deadline: June 1 of the year after you want to do your clinical rotation
• August - December deadline: February 1 of the year in which you want to do your clinical rotation

Along with this application, please include the following:
• A 200-word essay on where you see yourself professionally in 5 years
• Current CV/resume
• 2 professional references with contact information (phone and email address)

Only completed applications can be processed and sent through for consideration. Please submit all the requested information with this application.
