This position will be responsible for marketing, outreach, and education efforts to increase member enrollment in identified Counties. The Outreach Care Manager will have set expectations regarding referral generation and engagement activities to assist the Health Home program in reaching enrollment goals and community awareness across the various counties with hospitals, clinics, mental health providers, schools and Erie County stakeholders.Essential Job Duties:1. Develop a marketing plan with Health Home Supervisors and Leadership2. Represent Baker Hall at all relevant stakeholder meetings within Erie and Niagara County 3. Drive brand-awareness through on-site, face to face interaction with referral sources 4. Increase the number of referrals being sent to Baker Hall5. Provide community education regarding the Health Home program 6. Identify opportunities to collaborate with community-based organizations, providers, health systems, and participate in local events7. Provide staff education to internal and external departments as it pertains to the referral and engagement process8. Develop a deep understanding of local resources and services with the Buffalo and surrounding counties9. Must have a strong knowledge of Health Home Care Management services10.
