JOB SUMMARY:
The Care Navigator, under the direction of the primary care team, guides patients and their families through the health care system. Assists with access issues, and develops relationships with service providers and community resources. Assists patients and families with prescriptions, travel, and access to financial resources. Assists patients in connecting with community resources, including substance abuse treatment and elder care. Provides information on insurance benefits and co-pays. Guides patients through state and federal programs, and answers questions regarding patient insurance benefits.
QUALIFICATIONS:
Education
Associate’s degree in health sciences and/or medical records certificate program preferred.
Experience
Minimum of two years of experience in the health care field preferred.
Licensure/Credentials
Basic Life Support (BLS) certification.
