Healthcare Managed Care Community Connector Bilingual in Spanish (Remote NYC)

We are looking to hire a candidate with the experience and skill sets mentioned for one of our major clients within the Health Insurance industry. This is a REMOTE role. This is a 6-month contract opportunity. There is a possibility that the position will be temp to perm employment based on positive work.

The position is fully remote, but candidates MUST be local to New York City (Brooklyn, Queens, Bronx, or Manhattan

The schedule will be Monday to Friday, 8:30 am 5 pm EST.

*Bilingual with Spanish

Position Summary:

Community Connector primary focus is to conduct Outbound calls and Inbound to Molina members. Community connector tracking every month member’s health condition and reporting to Case manager. Community connector is assisting Molina members to be safe and happy in their community. Community connector uploading documents and assisting Case managers with task need it.

Position Responsibilities:

  • Experience with accessing to CCA, Salesforce, QNXT, HHAX, Genesys, HCS system, QCSS system, E-paces
  • Serves as a community-based member advocate and resource, using knowledge of the community and resources available to engage and assist vulnerable members in managing their healthcare needs.
  • Collaborates with and supports the Healthcare Services team by providing non-clinical paraprofessional duties in the field, including meeting with members in their homes, nursing homes, shelters, or doctor's offices, etc.
  • Empower members by helping them navigate and maximize their health plan benefits. Assistance may include:
    • Scheduling appointments with providers.
    • Arranging transportation for healthcare visits.
    • Getting prescriptions filled.
    • Following up with members on missed appointments.
  • Receiving Inbound calls from Queue 2 hours per day
  • 300 Outbound calls per month,
  • 35-40 calls attempted per day,
  • 15- 18 calls completed per day

Required Skills/Experience/Education:

  • 2 years of experience working with underserved or special needs populations, with varied health, economic, and educational circumstances.
  • Excellent communications skills, Basic computer skills, Professionalism and experience with speaking with elderly population.
  • Able to fluently speak Spanish, Researching and good speed typing skills
    Good listening skills with empathy
  • Candidate MUST have customer service skills and be empathetic as there will be a lot of contact/communication with members.
  • Candidate must be bilingual in English/Spanish
  • Must have a valid driver's license with a good driving record and be able to drive within the applicable state or locality with reliable transportation.
  • HS Diploma/GED, Associate or Bachelor s degree.

Other job specifications:

  • Employment Type: Contract to Hire (CTH), W2 only. NO C2C.
  • Contracting Period: 6-month contracting opportunity with the possibility for extension.
  • Job Location: The position will require traveling and visiting members. It is fully remote, but candidates MUST be local to New York City (Brooklyn, Queens, Bronx, or Manhattan).
  • The Schedule: Monday to Friday 8:30 am 5:00 pm EST.
  • Contracting Rate: $22/hr. on W2.
  • Interview Process: Microsoft Teams Meeting (1 interview).
