Job Overview:
We are seeking a dedicated Case Manager to join our team. The Case Manager will be responsible for providing support and guidance to individuals in need, ensuring they receive the necessary services and resources to improve their quality of life.

Duties:
- Conduct assessments to identify clients' needs and develop individualized care plans
- Provide counseling and support services to clients facing various challenges
- Coordinate with external agencies and service providers to ensure clients receive comprehensive care
- Monitor clients' progress and adjust care plans as needed
- Advocate for clients' rights and access to services
- Maintain accurate and up-to-date client records

Skills:
The ideal candidate should possess the following skills:
- Experience in addiction counseling and crisis intervention
- Proficiency in supervising and mentoring individuals
- Knowledge of child welfare services and childcare programs
- Familiarity with Section 8 housing assistance programs
- Background in social work, workforce development, and behavioral health
