Overview:
We are seeking a dedicated Registered Nurse Case Manager to join our team. The ideal candidate will have a strong background in emergency medicine, diagnostic evaluation, critical care, hospital medicine, acute care, and primary care. Experience in an ER or Level II Trauma Center is highly desirable.
Responsibilities:
- Provide comprehensive nursing care to patients
- Conduct diagnostic evaluations and assessments
- Collaborate with healthcare professionals to develop and implement patient care plans
- Monitor and evaluate patient progress
- Coordinate with interdisciplinary teams to ensure quality patient care
- Educate patients and families on treatment plans and self-care techniques
