In the capacity of a Field Registered Nurse (RN), you will serve as an indispensable component within an Integrated Care Model, tasked with the crucial responsibility of conveying pertinent member information and advocating on their behalf.

Primary Responsibilities:

• Assess, strategize, and execute individualized care plans tailored to each patient, with a focus on directing them towards the most suitable and least restrictive level of care.
• Utilize the New York State Uniform Assessment System (NYS UAS) Assessment, either in-home or via telehealth, as necessary, to formulate personalized service plans aimed at fulfilling the member's specific needs.
• Identify and instigate referrals for an array of social service programs encompassing financial, psychosocial, community, and state supportive services.
• Oversee the management of care plans across the continuum of care, serving as the central point of contact throughout the process.
• Effectively communicate vital health-related information to all stakeholders, ensuring the provision of expedited, high-quality coordinated care and services to all members.
• Advocate tirelessly for patients and their families when required, ensuring that the patient's preferences and requirements are fully represented and upheld by the healthcare team.

RN License Required
