Clinical Pharmacist
The Clinical Pharmacist at One Community Health works to facilitate comprehensive medication management and oversight for patients with chronic diseases, including but not limited to HIV, Hepatitis C, Diabetes, and Anticoagulation medication management and monitoring and other chronic conditions seen in the ambulatory care environment.
  • Provides patient care through one-on-one visits with patients.
  • Conducts medication reconciliation on all patient visits.
  • Provides mentorship for student and resident research projects.
  • Provides patient-specific analysis and consultations (oral or written) when appropriately requested.
  • Provides all aspects of pharmaceutical care to ambulatory patients.
  • Sees patients in the areas of their specialty and provides Comprehensive Medication Management
  • Take the lead on medication-related issues, including:
    • Evaluating complex medication needs for patients with co-morbid conditions
    • Drug-drug or drug-disease interactions
    • Titration of medications including de-escalation and opioid tapering consultations
    • Evaluating medication side effects, interactions, and incompatibilities
    • Recommend alternate drug entities based on insurance coverage issues as needed.
    • Recommend alternate routes of administration when appropriate
    • Provide education to patients and clinic
    • Adverse event and medication error reporting when necessary
    • Cost-saving initiatives
  • Order appropriate laboratory work for patients, interpret results and implement or recommend adjustments to medication therapy as outlined in collaborative practice agreements.
  • Oversee activities and supervise pharmacy technicians working in the clinical practice setting.
  • Participate in Quality Assurance by assuming responsibility for thorough clinical documentation, patient safety, improvements in clinical outcomes and for the development.
  • Implementation, and completion of quality improvement projects.
  • Maintain and expand professional expertise.
  • Achieve board certification or other specialty certification (ex. AAHIVP) in respective specialty area.
  • Maintain and complete all departmental and institutional training and certification requirements.
  • Understands current State and Federal regulations to maintain compliance and is remains knowledgeable on pharmacy policies, procedures, and clinical practice guidelines as participation in organizational surveys (Joint Commission, CMS, Board of Pharmacy) or other applicable regulatory or audit inspections may be required.

MINIMUM REQUIREMENTS
  • Must be California Pharmacist License eligible and possess a valid license at the time of hire.
  • Current California Board of Pharmacy license in good standing.
  • APhA Certified Immunizer
  • PGY-1 residency or three years of experience is required for this position.
  • Pharmacist Board Certification (e.g. BCPS, BCACP); certification in specialty area is preferred if currently available

  • Ability to supervise technicians and establish priorities of workflow.
